Home and Community Support Worker
Job Overview:
The Home and Community Support Worker is responsible for creating a warm and supportive relationship with the clients served by the agency fostering a sense of safety, belonging, and empowerment for each person. The Home and Community Support Worker facilitates opportunities for personal growth, self-determination, and provides experiences which support a valued role in the community for the people they serve. The Home and Community Support Worker effectively carries out the individual’s unique support protocols, encouraging maximum independence, self-worth, and safeguarding each client’s dignity and self-respect. She/he is expected to ensure that company policies and procedures are followed, individual’s goals are met, and excellence in quality of care is provided. This position requires demonstrated respect for the diversity and cultural identity of clients and peers, and a commitment to providing a person centred approach in all supports.
Reporting Relationship:
The Home and Community Support Worker reports to the Community Support Manager.
Key Duties & Responsibilities:
- conducts intake meetings with clients/family/caregivers ensuring complete documentation (both electronic and paper)
- provides person centred supports including life skills training as per the client’s personal support plan, facilitating experiences and community access opportunities that invite exploration, learning, and positive, interpersonal relationships.
- Support to clients requiring light housekeeping, cooking, and meal prep
- provides responsive, proactive, supports including responding to emergency situations, ensuring complete incident documentation post event
- assists with case management, identifying potential problems and reporting any difficulties.
- collaborates with professionals, families and natural supports involved with the client(s).
- engages in responsive, problem solving supports to clients and their families.
- identifies community-based opportunities and activities to address achievement of individual client needs and goals.
- maintains a healthy and safety environment, ensuring the safety of clients at all times.
- transporting clients either in employee vehicles, on public transit or using company van.
- maintains complete, professional documentation including, but not limited to:
- budgeting, payments, and banking
- daily tracking logs
- client documentation on goal achievement
- exit summaries
- demonstrated professionalism in interactions with clients, family members, and community contacts including punctuality, reliability, and adherence to policies and procedures, Code of Ethics.
- maintenance of agency facilities to health and safety standards including light cleaning
Hours and Conditions of Work:
The Home and Community Support Worker is expected to work up to 8 hours, per shift. Shifts include both home-based and community activities.
Home and Community Support Workers are required to be able to physically perform all of the duties associated with the care of clients, employing correct body mechanics, and including the use of safety equipment such as wheelchairs, and other mobility aids. The Home and Community Support Worker may be required to provide personal care, meal preparation, feeding, toileting, and will be required to actively participate in community programming, both outdoor and indoor activities.
Qualifications and Job Requirements:
Preference is given to candidates with a certificate or diploma in Community Support Worker, Human Service Worker, Special Needs Worker OR experience in the field.

- Valid B.C. Driver License and reliable vehicle available
- Dr.’s Note attesting to physical and mental capacity to perform the duties of the position
